Does your warcraftrealms id change if you name change in WoW? or race/realm change?

To my knowledge, yes, always, because after a change it's technically a different character.
As a registered user, you can however claim both the "old" and "new" character and add info on the "old" to the comment box of the "new" and vice versa.
